ACHIEVE Transition
ACHIEVE Transition is a post-secondary transition
option
for students with disabilities, age 18-21, referred from local,
partnering school districts. ACHIEVE Transition provides
individualized learning opportunities for students in an
age-appropriate, inclusive community college setting.
Each student will have access to general curriculum classes, ACHIEVE
courses, and college activities. With the assistance of staff,
students will make individual choices on college courses while
developing their employment skills in the community to make the
successful transition to adulthood with paid employment.
Upon completion of ACHIEVE Transition, each student will
have received the knowledge and experiences they need to be
gainfully employed and offered the supports required for long-term
sustainability.
ACHIEVE Transition will assist students and their
families in setting up support needs, schedules and transportation
requirements. Examples of transition activities include:

Paid and unpaid internships

College courses

Job sampling / Community-based assessments

Paid employment
